This form is to be used to report any safeguarding concern, disclosure, or incident involving a child, young person, or vulnerable adult during a Catalyst Youth Trust activity or event. Please complete the form as fully and accurately as possible, and submit it promptly. It will be seen by the Designated Safeguarding Lead. All information will be treated with sensitivity and in line with our Safeguarding Policy.
